I don't know whether you are "missing something" or not. But, there are many considerations to take into account as you make a purchase decision.
One is what I stated in an earlier post to this thread (in part):

Whatever these perks are, you wouldn't necessarily have them during your first three years of Clarity ownership without the Honda Care warranty.
Another consideration is that the Honda Care extended warranty is transferable to a new private owner if you ever sell the car, which I speculate will help support the future sales price and marketability of your Clarity at any time (unless you already know you will just trade the car to a dealer). And, since Honda's extended warranty is pro rata refundable anyway, the financial risk you take on by buying now is low.
Another consideration is simply peace of mind, which is a very individual thing. My personal preference has always been to "lock in" prices and other terms when I think they are good or at least reasonable. This applies to the D80 I purchased, for example, and extends even to the 3-year electricity fixed-price contract for our home (and Clarity) that I got from a local utility at $0.078 / kWh (the price is now (a year later) considerably higher).
I don't think you can make the wrong decision, either way. Like purchasing a Clarity in the first place, it is a personal decision for all of us, taking into account our personal situations, our own use cases, our assessment of the long-term reliability of the new technologies embodied in the Clarity, etc. The wisdom of our individual decisions on this subject (always to be evaluated in retrospect) most likely won't be known for some years.
