CA Hov Sticker for Clarity Electric Questions

Hi,
New Clarity Electric Leaser in CA here. Got my license plates and now completing my HOV decal DMV form which states my name must match DMV records. My registration has first and last name (and “lse” to indicate a lease). My drivers license has my first, last, and middle names. So, for the HOV form, is it correct to follow the registration (just first and last name)?

Second/third questions, when I return the car, do I need to remove the decal first? If yes, any good solvents to do that?

When I did mine I used my name as it appeared on my driver's license.

Last time I had a leased EV I just left the stickers on it when I returned it.
